Record Activity records when filling in an OT extension request

Is your feature request related to a problem? Please describe.
There was some confusing about when and who had started a request for an extension. Specifically, was it initiated through a form or the side-effect of detecting an email thread.

Describe the solution you'd like
A user should be able to click the "All comments & activity" link and see a log of everything that has changed about this feature entry, including how, when, and who requested an OT extension.

These records are not created manually. We need to add lines to every point in the code that makes changes.
