Help: I often get nipples on top of clothing, especially when upscaling

Hi guys,

I often have nipples generating on top of clothes like bikinis, tops, swimsuits etc. I have already tried putting all sorts of things in the negative prompt: nipples, nude, nipple, nudity, nsfw, topless, etc. but none of that really makes a difference.

The problem seems to mostly be caused by upscaling (latent) with high.res fix. Sometimes the image is fine, i.e. no nipples, without the upscaler, but then the upscaler paints them in (or sometimes even undresses the model). Also seems to be happening especially with light colored clothing, like white, pink etc. (which is closer to skin color, I guess).

I guess it happens because of a lot of nude training material, but could there be a good fix for this that any of you know of?

I've even tried to use a nipple remover LoRA that I found, but it didn't make any difference.
